THERAPEUTIC INDICATIONS: Due to its gastroenterokinetic, central and peripheral antiemetic and central analgesic properties, CARNOTPRIM® (metoclopramide) may be of therapeutic and/or prophylactic use in the following nosological entities:

CARNOTPRIM® (metoclopramide) is readily absorbed, either by gastrointestinal tract, or from intramuscular deposits; it reaches its maximum hematic concentration (Cmax), between 45 to 90 minutes, when administered orally; with the extended release formulation, the gradual intraluminal dissolution is released for 8 to 10 hours, amounts sufficient to maintain a stable state of absorption and therapeutic serum levels for 12 h.

The onset of the pharmacological action of metoclopramide is 1 to 3 minutes after intravenous administration, 10 to 15 minutes after intramuscular application and 30 to 60 minutes after oral administration; the bioavailability of metoclopramide is > 80%.

Its distribution follows a bicompartmental pattern, with a low binding to plasma proteins and a wide tissue diffusion, preferentially in the gastrointestinal tract, liver and biliary tract, in addition to the postrema area. Its metabolism gives by oxidation a monodesmethylated derivative, but most of it is conjugated giving glucide and sulfide derivatives, which are eliminated by urine or bile at the same time as the unchanged product, presenting an enterohepatic recirculation.

In sensitive patients, somnolence, tiredness and laxity may occur in the first days of treatment. Other reactions consist of sedation and dizziness, which in most cases disappear without the need to modify the therapeutic scheme or take additional measures. When the parenteral route is used, if the drug is applied quickly, it can cause extrapyramidal symptoms (torticollis, trismus, oculogyric crises), which are reversible when the medication is suspended or with the administration of diazepam or diphenhydramine.
